Scottsdale Lincoln Health Network (SLHN) is pleased to offer you online computer access to portions of your medical record through MyChart. MyChart is an Internet application that enables you to have secure web-based access to portions of your electronic health record, and allows a secure, convenient communications channel with SLHN affiliates for non-urgent messages. Use of this service is optional. MyChart and its related services are provided by SLHN affiliates to their patients subject to compliance with the terms and conditions set forth below. Please read the following information carefully. Your acceptance of these terms and conditions and your continued use of MyChart indicate your agreement with said terms and conditions. If you do not agree to be bound by these terms and conditions, promptly exit MyChart. Access to MyChart will only give you a limited picture of your medical records, if you wish to receive a complete copy of your medical records, please contact any Scottsdale Healthcare or John C. Lincoln Health Information Management (HIM) Department during normal business hours.
Your privacy is of the utmost importance to us. SLHN will use your confidential medical information in order to provide you with health care services. SLHN will, at all times, maintain your confidential medical information in strict confidence and will not disclose any of your information to any unaffiliated third party unless you authorize that party to receive your information or the information is permitted to be disclosed by law. Other services may be added to the MyChart web portal in the future. SLHN reserves the right to change, add and/or discontinue any/all MyChart services.
SLHN affiliate providers and/or staff may send you messages via MyChart. All communications between you and SLHN affiliates using MyChart occur over a secure connection. It is your responsibility to monitor these messages and respond in an appropriate manner. You must provide your SLHN affiliate with a valid email address and update SLHN affiliates if you change your email account. You may receive Internet e-mail messages notifying you that new information is available in your MyChart account. You should be aware that any person with access to your e-mail will be able to see this notification. This could include your spouse, employer or anyone else that can access your e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that you would not want others to know. Thus, you should take this into account when providing an e-mail address. You should not send SLHN affiliates messages via your personal email that contain personal health information as they may not be secure. If you do send SLHN affiliates an e-mail communication, it may be shared with the SLHN affiliate staff who assist the physician/provider with your medical care. Your confidential medical information in MyChart will be accessible only to appropriate SLHN affiliate staff. MyChart should never be used for urgent matters. The anticipated turnaround time for response to electronic messages is two business days. Therefore, if you believe you are experiencing an urgent or life threatening health condition, please contact your physician, go directly to an emergency department, or dial 911. If your SLHN affiliate provider is out of the office or unavailable, messages sent via MyChart may be routed to another provider in order to facilitate a timely response to your request.
SLHN employees are subject to strict standards on maintaining your confidentiality. By allowing you access to your medical information you must share this responsibility. Please read the SLHN Notice of Privacy Practices for more information. SLHN is dedicated to keeping your medical information confidential and SLHN affords the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by SLHN in any other medium. SLHN limits its employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, user provisioning, role-based access, and audit trails are further used to safeguard your information. SLHN shall identify the records released and note the time and date of access each time a patient or their approved Proxy accesses MyChart. Although MyChart is configured to be secure from unauthorized access, SLHN cannot be held responsible for:
Your login name and password are key to protecting the confidentiality of your information. It is your responsibility to keep your password confidential. It is only known by you and can be changed online at any time by you. The first time you log into MyChart you will be required to create a unique User ID and a six-character password. It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your confidential medical information. That person will also be able to read your messages and send new messages as if they were you. It is your responsibility to prevent disclosure of your login and password, and to change your password if you feel that your security has been compromised. You may change your password at any time, but it is recommended that you change your password every 180 days.
You may give access to your MyChart information to a personal representative of your choosing. Your Proxy will be responsible for complying with these same terms and conditions.
MyChart users must be 18 years of age or older to request a personal account, with the exception of emancipated minors who provide written documentation to prove their emancipated status. Parents or guardians of minor children may request access to the minor child’s account by completing the MyChart Child Proxy Form. Such access will only be granted to parties with parental rights or legal guardianship over the child and only to the extent that the party requesting Proxy access can demonstrate the legal right to account holder's medical information. Under State and Federal Law, there are certain types of medical information that the parent or guardian may not view without the consent of the minor patient. See your practice location for information regarding these limitations.
SLHN shall not be liable for any damages suffered by you or other third parties related to, directly or indirectly, the material and information contained in MyChart. Under no circumstances will SLHN be liable to you or any other person for direct, indirect, special, incidental or consequential damages of any type, even if informed of the possibility of such damages. SLHN cannot guarantee access to the MyChart system and is not liable for your inability to access or view the information contained in MyChart. By downloading or viewing the information on MyChart, you agree to hold SLHN harmless and indemnify SLHN against any damages, claims, or other claims asserted against SLHN arising out of, or in any way connected with, your utilization of MyChart or the information contained therein.
SLHN assumes no responsibility for how you use the information you obtain from MyChart. You should consult with a physician or other health care provider regarding your own condition and how MyChart content may or may not apply to you. Neither MyChart nor any other application available through MyChart is a substitute for appropriate and timely contact with your physician. You should never change or stop any course of treatment prescribed by your physician without first consulting him or her.
MyChart may not always be available due to system maintenance, back-ups or failures. Access is provided on an "as is available" basis and SLHN does not guarantee access at any particular time. If access is restricted for planned maintenance of MyChart, every effort will be made to communicate downtime to all users in advance.
You may disenroll from MyChart at any time by completing the appropriate online request to disenroll or by calling your provider’s office.
Access to MyChart is granted to users for the purpose of furthering their medical care. Use of the system unrelated to this purpose may result in the suspension or termination of access privileges. Improper activities include, but are not limited to, the use of inappropriate or abusive language, or frequently cancelled appointments and other activities that are deemed to cause a disruption to operations at the sole discretion of SLHN.
Although SLHN attempts to ensure the integrity of MyChart, it makes no guarantees whatsoever as to the correctness or accuracy of information contained in MyChart. It is possible that MyChart could include typographical errors, inaccuracies or other errors. In the event that you see an inaccuracy, please inform your practice so that it can be corrected. Information contained in MyChart may be changed or updated without notice.
SLHN makes no representations whatsoever about any other website that you may access through MyChart or any entity that may post its advertisements on MyChart. When you access a non-SLHN affiliate site, please be advised that it is independent from SLHN, and that SLHN has no control over the content on that website. SLHN does not endorse or accept any responsibility for the content or use of any other linked site. If you decide to access any third-party sites linked to MyChart, you do so entirely at your own risk.
This Agreement shall be governed by and construed in accordance with the laws of the State of Arizona.
The terms of this Agreement may be changed at any time at the discretion of SLHN. Updates to MyChart Terms and Conditions will be posted on MyChart for patient access.
